If you've ever tried to find senior resources for a parent or loved one, you know how it goes. You call one number, get transferred to another, end up on hold, and somewhere in that maze you lose an hour and still don't have an answer. There's no single place that lists everything — so most people either miss out on help they qualify for, or give up trying.
This guide is meant to fix that. We pulled together the most useful programs for seniors and families in South Sacramento and Elk Grove — organized by category, with phone numbers and websites, so you can find what you need without the runaround.
Senior Centers — A Good Place to Start
If you're not sure where to begin, a local senior center is often the best first call. They know the local landscape and can point you in the right direction.
- Senior Center of Elk Grove (SCEG) — 8230 Civic Center Drive, Elk Grove | (916) 685-3160 | thesceg.com
Fitness classes, social programs, health screenings, and on-site HICAP Medicare counseling. Open to anyone 50+. Most programs are just $2. - ACC Senior Services — accadvocates.org
A multilingual hub serving South Sacramento seniors. Offers transportation assistance, caregiver support, social programs, and connections to benefits. Well-regarded by families navigating the system for the first time. - Jose P. Rizal Community Center — South Sacramento (95823) | (916) 395-0601

Getting Around — Transportation Options in the Area
Transportation is often the single biggest barrier keeping seniors from the care and programs they need. There's actually more help available here than most people realize — including a brand-new benefit that launched in 2026.
- NEW in 2026: Elk Grove FREE Senior Transit Pass — Elk Grove residents age 62 and older can now ride SacRT buses at no cost. This program launched March 31, 2026, and is one of the most significant senior benefits to come out of Elk Grove in years. To enroll, call the City of Elk Grove at (916) 478-2200 or visit elkgrovecity.org.
- SacRT Paratransit (GO Service) — (916) 557-4685 | sacrt.com
Door-to-door ADA paratransit service for seniors and people with disabilities. Requires eligibility certification. $5 per ride. - SacRT Flex — South Sacramento Zone — sacrt.com/flex
On-demand flexible transit — you request a ride, it picks you up. Just $2.50 per trip for seniors 62+. A great option for the Franklin/South Sacramento area. - ACC Rides — accadvocates.org

Health Programs — Comprehensive Care Options
The programs below go well beyond a standard doctor visit. For seniors with complex needs, these can make an enormous difference in quality of life.
- InnovAge PACE Sacramento — 3870 Rosin Court, Sacramento | (866) 261-2954 | innovage.org
PACE (Program of All-inclusive Care for the Elderly) bundles medical, dental, vision, transportation, and adult day health into one coordinated program. Free for Medi-Cal eligible seniors. If your loved one has multiple health conditions and you're juggling multiple providers, PACE is worth a serious look. Most families who find it wish they'd found it sooner. - Sutter SeniorCare PACE — (833) 560-7223 | sutterhealth.org
Adult day health, in-home care, and transportation — also free for qualifying Medi-Cal seniors. Another strong PACE option in the Sacramento region. - Sacramento County IHSS — (916) 874-9471

Free legal help for adults 60 and older. Covers housing issues, benefits denials, elder abuse, and more. - Aging & Disability Resource Connection (ADRC) — (800) 211-4545

Getting There Is Half the Battle
Even with all of these resources available, getting to appointments and programs is still often the hardest part for families. If your loved one is on Medi-Cal, medical transportation is very likely a covered benefit — many families don't realize this. Call Modivcare at (855) 253-6863 to set up Medi-Cal-covered rides.
